Abstract

[fre] Dépenses médicales et couverture sociale . En France, la Sécurité sociale couvre la quasi-totalité de la population. Les différences de couverture du risque de maladie proviennent essentiellement des comportements d'adhésion à des mécanismes facultatifs de couverture complémentaire, dans lesquels les mutuelles tiennent une place prépondérante. En 1991, près de 80 % de la population disposait d'une assurance complémentaire. . Les non-mutualistes consomment moins que les mutualistes. Cette différence s'explique d'abord par l'état de santé et les caractéristiques socio-démographiques des assurés. Mais la consommation des non-mutualistes demeure plus faible même si l'on tient compte de ces effets. Il est possible que l'absence de mutualisation du risque et donc le coût de la participation aux dépenses incite les non- mutualistes à se montrer plus parcimonieux dans leurs consommations médicales. [eng] Medical Expenditure and Social Security Cover . In France, virtually the entire population has social security cover. The differences in health insurance cover are mainly due to the practice of signing up for optional supplementary cover, in which the mutual insurance associations hold a dominant position. In 1991, almost 80% of the population had supplementary insurance. . Those who do not hold mutual insurance cover consume less than those who do. This difference is initially explained by the state of health and socio-demographic characteristics of those with mutual insurance. However, the consumption of those without mutual insurance cover remains lower even when these effects are taken into account. It may be the lack of cover and therefore the cost of paying part of the expenses that prompts those without mutual insurance cover to be more frugal in their medical consumption. Unterschiede bei der Deckung des Krankheitsrisikos sind im wesentlichen auf das Verhalten hinsichtlich des Beitritts zu einem der freiwilligen Zusat- zversicherungssysteme zurùckzufûhren, bei denen die Hilfsvereine auf Gegenseitigkeit ("Mutuelles") einen be- deutenden Platz einnehmen. 1991 besaBen nahezu 80% der Bevôlkerung eine solche Zusatzversicherung. . Die Nichtmitglieder einer Versicherung auf Gegenseitigkeit nehmen weniger medizinische Leistungen in Anspruch als die Mitglieder einer solchen Versicherung. . Erklàren lâBt sich dieser Unterschied zunâchst durch den Gesundheitszustand und die soziodemographischen Ei- genschaften der Versicherten. Doch selbst bei Berùck- sichtigung dieser Faktoren nehmen die Nichtmitglieder von Versicherungen auf Gegenseitigkeit weniger medizinische Leistungen in Anspruch. Es ist môglich, daB das Fehlen einer zusâtzlichen Deckung des Risikos durch eine Versicherung auf Gegenseitigkeit und die somit anfallende Kostenbeteiligung dazu beitragen, daB sich die Nichtmitglieder solcher Zusatzversicherungen bei der Inanspruchnahme medizinischer Leistungen sparsamer verhalten. En 1991, casi el 80% de la poblaciôn era beneficiario de un seguro complementario. . Los no-mutualistas consumen menos que los mutualistas. Esta diferencia se explica primero por el estado de salud y las caracteristicas socio-demogrâficas de los afiliados. Pero sigue siendo menor el consumo de los no-mutualistas, aun si se consideran estas causas. Puede que la ausencia de mutualizaciôn del riesgo y que, por consiguiente, el coste de la participation a gastos inciten a que los no-mutualistas se muestren mâs parsimoniosos en sus consumos en servicios médicales.
