Abstract

Cette étude examine les effets du mode de financement des établissements hospitaliers sur la pratique de la césarienne en France en 2003. Nous exploitons la structure hiérarchique des données afin de mieux prendre en compte les différents facteurs influençant la pratique de la césarienne. Nos résultats montrent que les établissements privés, financés à l'acte, pratiquent significativement plus d'accouchements par césarienne que les établissements du service public, financés par enveloppe globale. La structure du personnel hospitalier a également un impact important sur la pratique ; plus le nombre d'obstétriciens par lit est important, plus la probabilité de recours à la césarienne est grande. De plus, nous constatons une différence de pratique non négligeable entre régions françaises.
