Wie lässt sich die gesetzliche Pflegeversicherung mit Hilfe personengebundener Budgets reformieren?

Abstract

Vor dem Hintergrund des steigenden Kostendrucks und eines Rekorddefizits der gesetzlichen Pflegeversicherung von 400 Mio. – in 2002 setzt die Rürup-Kommission in ihren Reformplänen für die gesetzliche Pflegeversicherung unter anderem auf die Erprobung so genannter personengebundener Pflegebudgets. Dabei handelt es sich um eine Geldleistung, die dem Pflegebedürftigen zum Kauf gewünschter Pflegedienstleistungen dient. Durch die Einführung dieser Leistungsform verspricht sich die Kommission die Ausschöpfung von Rationalisierungspotenzialen. Ziel dieses Beitrages ist es, nach einer ersten theoretischen Diskussion des Pflegebudgets, einen Überblick über bereits bestehende Ansätze zur Umsetzung eines solchen Konzeptes im nationalen und internationalen Kontext zu geben. Dabei wird der Frage nachgegangen, welche Voraussetzungen für eine erfolgreiche Erprobung eines PGB im deutschen Kontext gegeben sein müssen. Darüber hinaus identifiziert dieser Beitrag Forschungslücken, die es im Zuge der Erprobung eines personengebundenen Pflegebudgets zu schließen gilt. Zu diesem Zweck wird eine umfassende Evaluationsstudie empfohlen. Insbesondere wird angeregt, die Erprobung des Pflegebudgets als ein soziales Experiment zu konzipieren, bei dem neben einer Gruppe von Budgetempfängern auch eine Kontrollgruppe gebildet wird, um so eine adäquate Evaluation der Wirkungen des Pflegebudgets auf die Budgetempfänger vornehmen zu können. Darüber hinaus soll eine Erprobung des Pflegebudgets in verschiedenen Gemeinden auch eine Identifizierung indirekter Wirkungen im Gesamtsystem der pflegerischen Versorgung ermöglichen.
