Volkswirtschaftliche Kosten und Nebenwirkungen einer Bürgerversicherung

Abstract

Die Diskussion um die Reform des deutschen Krankenversicherungsmarktes kreist seit mehreren Jahren um die beiden Reformoptionen 'Gesundheitsprämie' und 'Bürgerversicherung'. Seit der Veröffentlichung des Berichts der sogenannten 'Rürup-Kommission' im Jahr 2003 wird die 'Bürgerversicherung' als eine Option für die Reform des deutschen Krankenversicherungssystems diskutiert (BMGS 2003). Diese Idee wurde insbesondere von den Parteien des linken politischen Spektrums (SPD, Bündnis 90/Die Grünen, Die Linke) mit jeweils unterschiedlichen Variationen aufgegriffen. Zentrales und namensgebendes Ziel der Bürgerversicherung ist es, einen integrierten Krankenversicherungsmarkt zu schaffen, der die bestehende Dualität aus der privaten Krankenversicherung (PKV) und der gesetzlichen Krankenversicherung (GKV) beseitigt. Dabei ist allen Bürgerversicherungskonzepten gemeinsam, dass hierzu die Zuständigkeit der GKV auf die Gesamtbevölkerung ausgeweitet werden soll. [...] Die vorliegende Studie beurteilt die volkswirtschaftlichen Effekte von Reformmaßnahmen in Bezug auf die PKV und quantifiziert sie, wo möglich, mit Hilfe eines Berechenbares Allgemeines Gleichgewichtsmodells. Das Modell bildet Wechselwirkungen zwischen den Eingriffen des Staates und den Reaktionen der Wirtschaftssubjekte ab und quantifiziert die Netto-Effekte auf das Sozialprodukt, die Beschäftigung und die gesamtgesellschaftliche Wohlfahrt. Unter Einbezug der prognostizierten Bevölkerungsentwicklung und Annahmen zur Produktivitätsentwicklung der Wirtschaft berechnet das Modell die Entwicklung der deutschen Volkswirtschaft bis 2040 und ermöglicht, auch mittel- und langfristige Effekte von Reformen zu erfassen. Zentrales Charakteristikum des Modells ist, dass es das Zusammenspiel aller Märkte abbildet und dass Haushalte und Unternehmen ihr Verhalten konsistent an geänderte Rahmenbedingungen (Preise, Steuern und Einkommen) anpassen. Darin unterscheidet sich diese Studie von anderen, die das Zusammenspiel der Märkte nicht abbilden.
