Mapeamento e caracterização dos estabelecimentos geridos por organizações sociais de saúde no Brasil: construção da Base de Dados das Organizações Sociais de Saúde (BDOSS)

Abstract

Social Health Organizations (OSS – Organizações Sociais de Saúde) are public-private partnerships for the autonomous management of healthcare service providers. Emerging from the administrative reforms and reduction of the state's role in the 1990s, they have shown relative success and have spread throughout Brazil. However, knowledge about OSS and the establishments whose management was transferred remains limited. This study addresses this gap by mapping and characterizing this landscape, with its main contribution being the expansion of Barcelos et al (2002) survey, incorporating methodological refinements. Manual research through websites and telephone centers of municipal/state health departments and information requests via the Access to Information Law (LAI – Lei de Acesso à Informação) was supplemented by information shared by the Federal Court of Accounts (TCU – Tribunal de Contas da União). The resulting database, named BDOSS, encompasses 1,874 establishments managed by 158 different OSS, and is available on an online portal that allows both direct searches in the database and the generation of statistics. All these procedures were monitored and validated through technical discussions with experts from the Brazilian Institute of Social Health Organizations (IBROSS – Instituto Brasileiro de Organizações Sociais de Saúde). The subsequent descriptive analysis of BDOSS shows that, despite the widespread presence, the participation of establishments managed by OSS is still low, with a higher concentration in São Paulo. The main types of services provided are in Primary Care. Regarding hospitals, which are the focus of the second stage of the study, there is a greater presence of OSS in medium/large-sized hospitals, with managed establishments showing a higher average volume of admissions and an occupancy rate with more efficient allocation of beds, surpassing the national average.
