Finanzierung des öffentlichen Gesundheitswesens

Abstract

In Österreich wird das öffentliche Gesundheitssystem zu einem großen Teil aus Beiträgen von der Lohn- und Gehaltssumme finanziert. Angesichts der Finanzierungsprobleme werden zunehmend alternative Finanzierungsmöglichkeiten diskutiert. Eine Anhebung der Höchstbeitragsgrundlage um 25% etwa würde jährlich Mehreinnahmen von 220 Mio. € bringen. Bei Aufkommensneutralität würde dies eine Senkung des Beitragssatzes um 0,4 Prozentpunkte erlauben. Eine Ausweitung der Beitragsgrundlage auf die Einnahmen aus Vermietung und Verpachtung brächte bei einem Beitragssatz von 7,5% rund 80 Mio. € an Einnahmen. Durch eine Umbasierung der Arbeitgeberbeiträge zur Krankenversicherung auf eine Wertschöpfungsabgabe könnte der Beitragssatz von 3,6% der Lohn- und Gehaltssumme (bis zur Höchstbeitragsgrundlage) auf 1,6% bis 2,6% der Wertschöpfung (je nach Abgrenzung) herabgesetzt werden. Eine Anhebung der speziellen Verbrauchsteuern (auf Mineralöl, Tabak, Alkohol usw.) um ein Viertel würde eine Verringerung des Beitragssatzes in der Krankenversicherung um 1¼ Prozentpunkte erlauben.
