Concurrence par comparaison et transformations quasi-marchandes

Abstract

En instaurant, à partir des années 1980, un calcul des coûts puis, au début des années 2000, une tarification hospitalière, les réformateurs français ont explicitement cherché à traduire dans les faits le principe dit de « concurrence par comparaison ». Nous montrons ici comment les régulateurs ont tenté d’incarner dans le calcul des coûts puis dans la tarification cette conception quasi-marchande, prise entre une conception libérale et une conception technocratique de la régulation hospitalière et en quoi ce mécanisme consiste. L’instauration de cette concurrence par comparaison suppose plus précisément une mise en équivalence du secteur public et du secteur privé afin de les rendre précisément « comparables » ce qui ne va pas sans poser certains problèmes quant à leur commensurabilité.
