Versorgungsqualität im DRG-Zeitalter: erste Ergebnisse einer qualitativen Studie in vier Krankenhäusern

Abstract

Mit der Einführung der Diagnose orientierten Fallpauschalen (DRG) haben sich die finanziellen Rahmenbedingungen für die Krankenhäuser in Deutschland verändert. Es wird befürchtet, dass die schon vor Einführung der DRG festzustellende Orientierung an wirtschaftlichen Zielen unter DRG-Bedingungen zunimmt und insoweit negative Folgen für die Patientenversorgung eintreten werden. Welche Folgen die DRG-Einführung für die Arbeitsbedingungen der Beschäftigten im Krankenhaus und die Qualität der Patientenversorgung hat, wird im Projekt Wandel von Medizin und Pflege im DRG-System (WAMP) untersucht, das vom Zentrum für Sozialpolitik in Zusammenarbeit mit dem Wissenschaftszentrum Berlin durchgeführt. wird. In diesem Beitrag werden erste Ergebnisse der qualitativen Teilstudie von WAMP vorgestellt. Es wird untersucht, wie sich die DRG-Einführung aus Sicht der von uns interviewten Krankenhausleitungen, Ärzte und Pflegekräfte auf die Patientenversorgung auswirkt. Dabei wird auch kurz darauf eingegangen, ob es sich um eine einheitliche Entwicklung handelt oder ob es Spielräume bei der Umsetzung der Anforderungen des DRG-Systems gibt bzw. ob Unterschiede zwischen oder sogar innerhalb einzelner Krankenhäuser festzustellen sind.
