Kostenbeteiligungen, Wartezeiten, Leistungsumfang - Ein europäischer Vergleich der Gesundheitssysteme

Abstract

Im Vergleich von europäischen Gesundheitssystemen zeigen sich deutliche Unterschiede im Zugang zu Gesundheitsleistungen. Deutschland schneidet mit seinem umfangreichen Leistungskatalog, den geringen Wartezeiten und der ausgeprägten Patientenautonomie bei der Wahl eines Arztes oder Krankenhauses im Ländervergleich sehr gut ab. Insbesondere in steuerfinanzierten Ländern stellen Wartezeiten dagegen ein sehr relevantes Versorgungsproblem dar. Es wird dabei deutlich: Einheitlich kollektiv finanzierte Gesundheitssysteme bieten keinen Schutz vor Versorgungsunterschieden. Die WIP-Studie zeigt darüber hinaus auf, dass insbesondere in einheitlichen Systemen Leistungen leichter gekürzt werden können. Einige Länder etablierten zudem Hürden, indem sie durch Gatekeeping die Wahlfreiheit einschränken. Eine Möglichkeit, einen niedrigschwelligen Zugang zur Versorgung aufzubauen, könnte über die Digitalisierung vorangebracht werden. So könnten kleinere Angelegenheiten deutlich schneller auf dem Weg gebracht werden und die Abstimmung zwischen den Akteuren effizienter gestaltet werden. Hier liegt in einige Ländern, auch in Deutschland, noch Potenzial brach.
