Abstract
O objetivo deste texto é o de discutir a relevância da produção e do uso da informação de custos do Sistema Único de Saúde (SUS) como insumo para os processos decisórios sobre o seu financiamento e gestão. Parte-se de análise da literatura técnico-científica sobre o tema para apresentar conceitos e discutir questões metodológicas envolvendo a apuração e a gestão de custos em saúde. A relevância da informação de custos para o SUS é discutida sob dois aspectos: o financiamento e a gestão. No que se refere à gestão de custos em saúde, são abordadas duas óticas para o uso da informação, a do gestor, nas três esferas de governo; e a do gerente das unidades de saúde. Apresentam-se experiências internacionais quanto a produção da informação de custos e seu uso na tomada de decisão em saúde para ilustrar algumas possibilidades que podem ser consideradas para o caso brasileiro, bem como experiências nacionais desenvolvidas por hospitais e órgãos do governo federal. Por fim, recomendações para a produção e uso sistemáticos da informação de custos no SUS são apresentadas como reflexões iniciais e subsídios para a formulação de uma política nacional sobre o tema. The objective of this text is to discuss the relevance of the production and use of cost information from the Brazilian Public Health System (SUS), as an input for decision making processes on its financing and management. Technical-scientific literature on the subject is considered to present concepts and discuss methodological questions involving the costing and health cost management. The relevance of cost information to the SUS is discussed under two aspects: financing and management of the system, dealing with two perspectives for the use of information, that of the system managers at the three government spheres and that of the health units’ managers. International experiences on the production and use of cost information are presented to illustrate some possibilities that may be considered for the Brazilian case, as well as national experiences developed by hospitals and federal government agencies. Finally, some recommendations for the systematic production and use of cost information in SUS are presented as initial reflections and subsidies for the formulation of a national policy on the subject.
