混合診療禁止制度の経済分析

Abstract

本稿の目的は，日本の医療制度において保険適用外診療の供給体制を規定する混合診療禁止制度について，経済理論的な評価を行うことにある．混合診療禁止制度とは，初診から治療の完了に至る一連の診療過程において，保険診療と保険適用外診療との併用，つまり混合診療を原則禁止とする制度である．混合診療禁止制度のもとで混合診療を患者が受診するならば，保険診療に該当する診療にも保険適用が認められず，一連の診療によって生じる医療費の全額を患者が自己負担することになる．混合診療禁止制度は，国民皆保険制度のもとで，保険適用外診療の一般化が，所得格差を通じた受療機会の不平等を招来することを防ぐ仕組みとして従来正当化されてきた．しかしながら，社会経済が豊かになり患者の価値観が多様化し，医療技術が急速に進歩している現在において，混合診療禁止制度による保険適用外診療の供給体制を巡って多くの議論が活発に行われている．本稿では，このような混合診療禁止制度の特性を定式化し，混合診療禁止制度の持つ制度的特性および制度的効果を明らかにし，医療制度における保険適用外診療の供給体制のあり方について考察する．特に，現在主に高額診療に対して適用されている特定療養費制度の適用範囲を，より必需的かつ低額な診療に対して拡張することによって混合診療を容認する制度へ移行することが，純粋多数決ルールのもとで社会的に是認される状況を特定する．本稿で示される主要な結論は，以下の3点である．1. 社会経済が豊かになるほど，もしくは低額な診療であるほど，混合診療を容認することが社会的に是認されやすくなる．2. 最低所得水準にあるような患者が，公的保険適用範囲の限界水準にある診療を受診できる場合には，混合診療を容認することによって，むしろ受療機会の平等性は改善する．3. 混合診療を容認することによって，新しい医療技術の普及を促進することができる．
