Abstract

Cet article vise à analyser la variabilité des coûts hospitaliers à partir du Programme médicalisé des systèmes d'information (PMSI). Il évalue la variabilité résiduelle des coûts des séjours hospitaliers pour infarctus du myocarde aigu, une fois tenu compte du classement en groupes homogènes de malades (GHM). Il évalue en particulier la part de variance résiduelle expliquée par la prise en compte d'actes non pris en compte dans la classification du PMSI tels que l'angioplastie, technique moins invasive, qui peut être pratiquée préventivement et permet de préserver la qualité de vie du patient. Il montre que les caractéristiques individuelles des patients et les actes non pris en compte dans la détermination des GHM, outre la durée moyenne de séjour, réduisent de façon significative la variance résiduelle des coûts des séjours étudiés.
