Abstract

La réforme de 2007 a créé un nouveau rôle, à l'hôpital, celui de chef de pôle. Ce médecin gestionnaire au rôle « hybride » est supposé pouvoir concilier les tensions entre la clinique et la gestion. Encore faut-il que les médecins endossent ce nouveau rôle. Notre travail s'appuie sur une recherche conduite pendant trois ans au sein de trois hôpitaux de province. Si la réforme prescrit un rôle hybride, les chefs de pôles se déclarent « médecins, avant tout », signe d'une identité qui reste profondément médicale. Une étude plus précise des prises de position lors des réunions montre toutefois que ces mêmes médecins se réapproprient des logiques économiques. Le passage du rôle déclaré au rôle joué est largement expliqué par le contexte de pression budgétaire, ainsi que par les instruments de gestion et les interventions de tiers. L'identité des chefs de pôle reste ainsi médicale, mais ces médecins sont « enrôlés » dans la gestion par le contexte d'action. L'hybridation est alors une affaire de « bricolage individuel » marquée par les parcours individuels et les contextes locaux de l'action, davantage qu'elle n'est affaire d'organisation
