L'utilisation de la science pour formuler la politique d'exemption du paiement des soins de 2016 au Burkina Faso

Abstract

En mars 2016, le gouvernement nouvellement élu du Burkina Faso a décidé d'un changement majeur dans la politique de financement de la santé : il a supprimé le paiement direct des soins de santé pour les femmes et les enfants de moins de cinq ans. Cet article décrit ce processus politique et présente une analyse réflexive de deux auteurs qui ont été au coeur des événements entre 2008 et 2018. L'analyse montre que, si la décision a pris beaucoup de temps et a certainement représenté un changement de paradigme politique, elle a été le résultat d'une série complexe d'événements et d'activités dont les contributions spécifiques sont difficiles à identifier. Le financement à long terme de projets pilotes visant à tester le nouvel instrument politique, associé à la production de preuves mobilisées par une myriade d'activités de transfert de connaissances, a été un élément crucial de la décision. En outre, il a fallu la mobilisation continue de coalitions de défense, des actions visant à contrer les idées préconçues sur cet instrument et l'émergence d'une fenêtre d'opportunité essentielle -le soulèvement populaire de 2014- pour que la décision soit possible.
