Abstract

A travers l'étude de l'évolution du système sanitaire français depuis les années 1960 nous montrons que la montée en puissance progressive du territoire régional comme échelon de référence, sous l'effet d'une intervention grandissante de l'Etat, aboutit aujourd'hui à une contractualisation généralisée de l'action publique en santé centrée autour des ARS. Contrairement à ce qu'affirment ses promoteurs, la contractualisation est loin d'être l'aboutissement de la décentralisation du système sanitaire ou la garantie de l'autonomie de ses acteurs, le contrat comme mode de coordination se révélant être en réalité l'instrument d'une nouvelle étatisation ouvrant la voie à la marchandisation de la santé.
