Abstract

En 2008, 46,3 millions d'Américains étaient non assurés dont près de 1 enfant sur 10 et 1 travailleur sur 5. Pourtant, les salariés doivent être couverts par une assurance santé dans le cadre de leur entreprise tandis que les personnes de plus de 60 ans, les populations les plus démunies et les enfants de moins de 18 ans bénéficient des programmes de financement public. Mais la cherté des contrats d'assurance rend difficile l'accès aux soins pour une partie croissante de la population notamment de la classe moyenne et de celle travaillant dans des petites entreprises. Signé le 21 mars 2010 par Obama, le Patient Protection and Affordable Care Act doit mettre fin à ces inégalités. Cette nouvelle loi doit garantir à tous les résidents légaux américains, l'adhésion à une assurance santé grâce à un système d'incitations-sanctions (subventions ou pénalités). Elle simplifie les modalités d'adhésion aux programmes publics et accroit la transparence et la concurrence entre compagnies d'assurance. Mais la réforme ne traite pas véritablement la question de la maîtrise des dépenses de santé.
