I farmaci ospedalieri tra Europa, Stato, Regioni e Cittadini

Abstract

Dopo il rilascio dell'Autorizzazione all'Immissione in Commercio, un farmaco "H", prima di divenire concretamente disponibile in ospedale, deve completare una trafila che può differire da Regione a Regione, e addirittura, all'interno di una stessa Regione, da ASL ad ASL o da ospedale ad ospedale. Tra l'autorizzazione nazionale e la possibilità per il medico ospedaliero di utilizzare il farmaco si possono frapporre commissioni territoriali o locali di vario livello che presiedono ad altrettanti prontuari, con potere di filtro o addirittura di blocco per l'area di loro competenza. Nei casi più "fortunati" documentati nel Quaderno, uno stesso farmaco diviene effettivamente utilizzabile nei DRG a date diverse a seconda della Regione, della ASL, della struttura ospedaliera. In altri casi, le commissioni locali possono arrivare a bloccare del tutto l'approdo di un farmaco nei DRG, così differenziando strutturalmente il Prontuario Farmaceutico Nazionale dai prontuari in uso sul territorio. L'approvazione dell'EMEA e/o dell'AIFA assicurano che i farmaci autorizzati all'immissione in commercio abbiano efficacia terapeutica.
