Incidencia de los gastos de bolsillo en salud en siete países latinoamericanos

Abstract

Para los casos de Argentina, Brasil, Chile, Colombia, Ecuador, México y Uruguay se caracterizan los niveles de gasto de bolsillo en salud (GBS);. Argentina resalta como un país con una cobertura de seguro social de salud relativamente extendida, pero en el cual los hogares deben desembolsar grandes sumas (en relación a su capacidad de pago); en concepto de gastos de bolsillo. El 10% de la población con mayor gasto tiene erogaciones que superan el 20% de su capacidad de pago total, valor muy superior a los de Chile y Ecuador, que son 10% y 7.5% respectivamente. En Uruguay, los hogares realizan GBS relativamente moderados, pero tienen gastos de bolsillo relativamente importantes por seguros médicos. En los siete países, tres características elevan la incidencia de los gastos de bolsillo: jefe mayor a 65 años, eventos hospitalarios, presencia de adultos mayores. Los gastos catastróficos están asociados a eventos hospitalarios. Los gastos de bolsillo por eventos hospitalarios segmentan a los países en tres grupos. En Chile y México un hogar que experimenta un evento hospitalario puede ver crecer su razón gasto a capacidad de pago en 5 y 6 veces respectivamente. En Argentina, Brasil, Colombia y Ecuador, esta razón crece entre 2 y 3 veces. Finalmente, en Uruguay, un hogar que experimenta un evento hospitalario ve aumentar su razón de gasto a capacidad de pago en apenas un 20%.
