Améliorer les parcours de diagnostic du cancer du sein au Québec

Abstract

In 2022, Canada saw nearly 28,900 new cases of breast cancer, making it the most prevalent cancer among women and the second most common across the entire Canadian population, following lung cancer. Delays in diagnosis can significantly worsen the condition and amplify existing inequalities. Despite Quebec setting a 17-day investigation timeframe after an abnormal mammography result back in 1999, the CEO of the Quebec Breast Cancer Foundation bemoaned in October 2021 that the actual delay could sometimes extend up to 17 weeks. Drawing from scientific literature on cancer care, government reports, and expert interviews, this document delves into Quebec's diagnostic capabilities and performance. The authors compare breast cancer diagnostic innovations in Quebec with those in Alberta and Ontario, where diagnostic delays are comparatively shorter. They present practical strategies to accelerate and streamline the breast cancer diagnostic process, guiding readers toward essential resources for deeper insights. The authors recommend integrating high-risk individuals into Quebec's organized screening program, customizing the screening process, modernizing available imaging technologies and genetic tests, and enhancing communication methods. Il s’agit du cancer le plus fréquent chez les femmes et du deuxième cancer le plus courant parmi l'ensemble de la population canadienne, derrière le cancer du poumon. Les retards de diagnostic peuvent aggraver la maladie et intensifier les inégalités. Pourtant, malgré le délai d’investigation de 17 jours après un résultat anormal à la mammographie fixé par le Québec en 1999, la présidente-directrice générale de la Fondation du cancer du sein du Québec déplorait en octobre 2021 un délai effectif «[pouvant] parfois atteindre jusqu'à 17 semaines.» Fondé sur la littérature scientifique sur les soins du cancer, les rapports gouvernementaux et des entretiens avec des experts, ce document explore les capacités et performances du Québec en matière de diagnostic. Les auteures comparent les innovations en matière de diagnostic du cancer du sein au Québec à celles de l’Alberta et de l’Ontario, où les délais de diagnostic sont plus courts. Elles présentent des stratégies concrètes pour accélérer et rationaliser le processus de diagnostic du cancer du sein et identifient des ressources essentielles pour approfondir les réflexions. Les auteures préconisent d’intégrer les personnes à risque élevé dans le programme de dépistage organisé du Québec, de personnaliser le dépistage, de moderniser les technologies d’imagerie et de tests génétiques disponibles, ainsi que d’améliorer les méthodes de communication.
