Preliminary Hazard Analysis (PHA)

Abstract

The preliminary hazard analysis (PHA) method is explained and a worked example provided to support the explanations. Some extensions to the original method are also mentioned. Among these extensions, the PHA including the determination of the frequency of the potential accident is of common use.
