Abstract

Zusammenfassung In diesem ersten Kapitel stellen wir als Einleitung und Illustration der Problematik hinsichtlich der menschlichen Fehler im Zusammenhang von komplexen Systemen drei berühmte und signifikante Beispiele von fatalen Irrtümern vor: Das Flugzeugunglück von Teneriffa, der Unfall auf der Nordsee-Ölplattform Piper Alpha sowie der Chemieunfall von Seveso. Bei jedem dieser Desaster konnte festgestellt werden, dass aufgrund von menschlichen Fehlhandlungen jeweils eine Fehlerkette ausgelöst wurde, die in fine zu der Katastrophe mit all ihren Konsequenzen geführt hat.
