Abstract

Zusammenfassung In der regionalen Integrierten Versorgung stellt eine systematische Organisation von sozialen, pflegerischen und medizinischen Daten die zentrale Plattform zur Verbesserung und Effizienzsteigerung in der Gesundheitsversorgung bzw. der Prävention vor Ort. Die Interoperabilität der Daten bildet dabei die Grundlage für die Zusammenarbeit und den nahtlosen Austausch zwischen den Leistungserbringern untereinander und mit möglichen Koordinatoren in der Region wie bspw. Gesundheitskiosken oder -regionen. Das Krankenhaus kann dabei entweder als wichtiger Leistungserbringer agieren oder auch selbst die Trägerschaft für die Koordinatorrolle übernehmen. Mit der richtigen Strategie können Entscheidungsfindung, Abstimmungsprozesse und Datenqualität beschleunigt bzw. verbessert werden.
