Abstract

Zusammenfassung Die Krankenhauslandschaft in Deutschland befindet sich in einer kalten Transformation. Diese beruht überwiegend auf einer markt- und wettbewerblichen Steuerung mit lokalpolitischer Einflussnahme. Übergeordnete versorgungsökonomische Zielsetzungen setzen sich nur selten durch. Die Dynamik der Transformation wird durch die notwendige Digitalisierung, die weitere Ökonomisierung und die zunehmende Ambulantisierung des ersten Gesundheitsmarktes beschleunigt. Der Wechsel zu einer strukturierteren Transformation ist notwendig, um diese Entwicklungen sowie die demografischen und ökonomischen Herausforderungen ohne enorme Belastungen der Beitrags- und Steuerzahler oder Qualitätsverluste zu bewältigen. Um dabei die Subsidiarität und Trägervielfalt, die das deutsche Sozial- und Gesundheitssystem über 100 Jahre stabilisiert und weltweit führend gemacht hat, zu erhalten, sind gleiche Bedingungen für öffentliche, freigemeinnützige und private Träger zu schaffen. Bund und Länder sind gefordert, grundsätzliche Entscheidungen zur zukünftigen Krankenhausfinanzierung und -planung zu treffen.
