Abstract

A three-type model of health care delivery is constructed, in which individuals seek care at a primary care station and may be referred to a clinic or a hospital. The probability of seeking care depends on symptom level. (The demands for medical care depends on cost, ease of access, perception of the efficacy of care, and symptom level; the first three are represented by shifts in the probability of seeking care in the model.) Each provider has efficacy and referral matrices; individuals decide whether to pursue care. The model is simulated to investigate the cost-heahh status trade-offs. The outcome measure is used to investigate the efficacy of social policies, such as increasing access to the system. The cost-health status frontier is insensitive to a wide range of schemes for weighting symptom levels. The underlying health status of the population is much more important than the quality of medical care in determining the health status.
