Transferbedarf in einer prämienfinanzierten Gesetzlichen Krankenversicherung

Abstract

Die Gesetzliche Krankenversicherung wird im Jahr 2010 mit 15,7 Milliarden Euro aus Steuermitteln subventioniert. Dadurch sinkt der Beitragssatz nicht nur für einkommensschwache, sondern auch für leistungsfähige Mitglieder von 16,5 auf 14,9 Prozent. Statt die Steuerzahlungen pauschal einzusetzen, sollte der Staat sie bedarfsgerecht verteilen, um den Wechsel von der Beitrags- auf die Prämienfinanzierung sozial abzufedern. Werden sämtliche Ausgaben über eine monatliche Prämie von 250 Euro je Erwachsenen finanziert, muss der Steuerzahler statt der aktuell 15,7 Milliarden Euro zwischen 16,3 und 11,2 Milliarden Euro aufwenden, um unzumutbare Belastungen zu kompensieren. Unerwünschte Mitnahmeeffekte, die im Status quo auftreten, lassen sich dabei vollständig vermeiden. Wird dagegen nur ein Teil des bisherigen Arbeitnehmerbeitrags in eine Prämie gewandelt, sinkt zwar die Nettobelastung für den Staat bis auf unter 7 Milliarden Euro. Die Fehlverteilungen bleiben aber bestehen.
