Проблемы Перехода На Подушевой Принцип Оплаты Первичной Медико-Санитарной Помощи

Abstract

В статье рассматривается государственная политика в отношении способов оплаты первичной медико-санитарной помощи в системе обязательного медицинского страхования. Обосновывается необходимость перехода от оплаты посещений врачей к подушевой оплате первичной медико-санитарной помощи. Подвергаются анализу и оцениваются последние нормативные документы по данному вопросу. Предлагаются альтернативные подходы, направленные на сдерживание затрат и усиление профилактической направленности деятельности поликлиник, в том числе метод фондодержания. Рассматриваются методические проблемы формирования норматива подушевой оплаты. Рассматривая возможные риски фондодержания, автор предлагает к обсуждению методы их демпфирования. В качестве возможного варианта изменения метода планирования амбулаторной помощи в статье предложен переход от планирования посещений врачей к планированию, исходящему из численности обслуживаемого населения, нормативов расходов на одного жителя и показателей результатов деятельности поликлиник. В заключение автор приводит алгоритм перехода на подушевой метод оплаты первичной медико-санитарной помощи.
