L?esito delle cure specialistiche e riabilitative integrate di multi-patologie e cronicit?

Abstract

L?utenza sanitaria a seguito dei cambiamenti epidemiologici e sociali conseguenti all?aumento della vita media ? caratterizzata da pazienti anziani, cronici e comorbidi, che ricorrono a ricoveri ripetuti e spesso con esiti disabilitanti. In questo contesto diventa indispensabile un approccio sia clinico che gestionale innovativo che promuova sistematicamente con cure integrate l?autonomia della persona. Le medicine specialistiche e riabilitative sono, come evidenzia l?esperienza qui presentata, la risposta appropriata a questo obiettivo perseguita con percorsi interdisciplinari di cure correlate delle malattie disabilitanti, diagnosticate biologicamente e funzionalmente con ICD-ICF. Definizione e personalizzazione dei percorsi/protocolli, valutazione degli esiti sono le fondamenta di questo nuovo modello clinico realizzabile grazie alla gestione digitale dei dati diagnostici e terapeutici del paziente e delle conoscenze mediche.
