Injuria pulmonar aguda. Características generales. Cuba 2018

Abstract

La injuria pulmonar aguda es una respuesta del pulmón a múltiples estímulos sistémicos o locales, cuyo espectro abarca el edema pulmonar no cardiogénico y al síndrome de distrés respiratorio agudo. Su patogenia no está perfectamente aclarada y se señala que los macrófagos fijos del pulmón inician el proceso con liberación de citoquinas y activación de los sistemas de coagulación, complemento fibrinolíticokinina-kalikreinas, del metabolismo del ácido araquidónico y de agentes proteolíticos y oxidantes con producción de daño hístico. Se expone la evolución de los criterios diagnósticos establecidos y se señala que hasta la fecha, el tratamiento de la enfermedad de base y las medidas de sostén constituyen las alternativas terapéuticas efectivas.
