Effektivitäts-, Effizienz- und Qualitätsreserven im deutschen Gesundheitssystem

Abstract

Die Gesundheitsausgaben sind in Deutschland vergleichsweise hoch. Die WHO hat den Output und die Performance (Verhältnis von Input und Output) des deutschen Gesundheitssystems als nur durchschnittlich beurteilt. Auch in den Qualitätsindikatoren der OECD schneidet das deutsche Gesundheitssystem durchweg bestenfalls durchschnittlich ab. Die Ursachen für Über-, Unter- und Fehlversorgung - und damit auch für das Vorliegen nennenswerter Effizienz-, Effektivitäts- und Qualitätsreserven im deutschen Gesundheitssystem - sind vielfältig. Vor allem lassen jedoch sektorspezifische Budgets und kollektivvertragliche Steuerung wenig Spielraum für die Realisierung von Effizienzreserven.Vor diesem Hintergrund schlagen die Autoren dieser Studie vor, den Vertragswettbewerb auf der Leistungsseite zu stärken. Das GKV-WSG sieht durchaus einige Maßnahmen zur Stärkung von Vertragswettbewerb vor, doch gibt es auch Maßnahmen, die diesem Ziel entgegenstehen. Insbesondere ist ein halbwegs geschlossenes Wettbewerbskonzept nach wie vor nicht erkennbar. Die im GKV-WSG beschlossenen Maßnahmen sind damit nur sehr eingeschränkt dazu geeignet, die Leistungsfähigkeit des deutschen Gesundheitssystems zu erhöhen.
